PEOPLE can hear about progress on developing Care Communities in the area by attending East Cheshire NHS Trust’s annual meeting.

Members of the public, stakeholders and trust staff are welcome to attend to hear about the trust's activity and performance during 2017/18 on a day which also marks the 70th anniversary of the NHS.

The AGM will hear about progress in developing Care Communities which are being established across the area to deliver better out-of-hospital care to patients and prevent unnecessary admissions to hospitals.

The Care Communities involve people from primary, health and social care working together to deliver a streamlined service with the patient at its heart.

The aim is for patients to experience ‘more seamless care’ closer to their homes, delivered by health and social care professionals who understand their needs.

Five Care Communities have been formed so far, based on their geography and population size - Knutsford; Bollington, Disley and Poynton; Macclesfield; Congleton and Holmes Chapel; Chelford, Handforth, Alderley and Wilmslow.

The annual meeting will take place at 1.45pm on Thursday, July 5 on the Macclesfield Hospital site.

The AGM will be followed by the monthly meeting of the trust’s board, which the public are also welcome to attend.

The board meeting takes place from 3pm to 5pm in the same venue - Boardroom 1, New Alderley House, Macclesfield District General Hospital.
